Kaiser Permanente of Ohio offers a number of health plans for its customers:

Individual and family plans: These co-payment plans are suitable for families with small children or others who make regular visit to doctor. Size of co-payment determines monthly rate: Plan 20/20 and Plan 25/100. Its deductible plans are best in case you do not often need a doctor but wish to cover large health expenses. These plans are available in three ranges: Plan 500/1000, Plan 750/1500 and Plan 1000/2000.

Its HMO program includes all the health and prescription coverage you expect from a health care plan. Employees get excellent care and service, convenience, low co-payments, no deductibles, and no claim forms, and all at reasonable prices. HMO coverage includes outpatient services, hospitalization, medically required treatments, and ongoing preventive care coordinated through the primary care physician you have chosen and the physician's primary care team.

Kaiser Permanente’s Medicare Plus plans enhance your retirees' Medicare coverage. These plans offer health care coverage, which includes outpatient services, hospitalization, preventive care with no deductibles or claim forms when care is received within our service area, and low co-payments.

Its Added Choice is a quality health care plan that allows your employees and their families to choose from four benefits options. Members can choose to see a Kaiser Permanente or affiliated physician, a physician from the company’s preferred provider network, or any out-of-network physician or hospital they desire. It is the HMO with the added flexibility of a preferred provider organization (PPO).

Kaiser Permanente has entered into contracts with community-based physicians and other medical professionals in Cuyahoga County (MetroHealth network), Lake County (Lake County network), Lorain County (the EMH Regional Network), Medina County (Medina County network), and Stark and Summit counties (the Southern network). These providers, together with the Ohio Permanente Medical Group physicians who are based at the Kaiser Permanente medical offices, are referred to as the Kaiser Permanente provider network. All these physicians have been listed in the company’s provider directory.
